The TMF Home Health Agency Quality Improvement Award program recognizes agencies in the states of Texas, Arkansas, Missouri, Oklahoma and the U.S. territory of Puerto Rico that meet or exceed specific outcome measures. The criteria include measures related to TMF’s contract with the Centers for Medicare & Medicaid Services. All home health agencies meeting or exceeding the criteria will receive an award.
Home Health Agency Award performance measures begin May 1, 2016, and end Dec. 31, 2017. TMF will announce the award recipients in the third quarter of 2018. Award recipients will receive a commemorative certificate, recognition through a media campaign and announcements in various trade publications.
Any home health agency in the state of Texas, Arkansas, Missouri, Oklahoma and the U.S. territory of Puerto Rico that demonstrates improvement in the outlined criteria over a 20-month time frame will be recognized with a quality improvement award. The agency must not have a condition of participation-level deficiency on its last Medicare certification survey. View the program overview for more information. (PDF)
What determines an award winner?
The 2016 – 2017 TMF Home Health Agency Quality Improvement Award criteria consist of three major components:
- Exemplary performance on nationally recognized clinical outcome measures
- Regular use of blood pressure measurement and control (self-assessment and submission)
- Registration and data input into the Home Health Quality Improvement Cardiovascular Data Registry for a minimum of six months

What is the process and timeline for the award program?
- May 1, 2016 - Dec. 31, 2017: Quality Improvement Period
Home health agencies focus on providing exemplary care and to meet or exceed the established criteria.
- July 2016 - August 2018: Home Health Agency Performance Measure Reporting
Data is pulled for all home health agencies based on the established award criteria and the current status is uploaded to the QualityNet inbox on a quarterly basis.
- Third Quarter 2018: Review Period
Final data will be reviewed for all home health agencies to decide which facilities met the criteria for a Gold, Silver or Bronze award.
- Third Quarter 2018: Award Recipients Notified
Home health agencies will be notified as to whether or not they have met the criteria to receive a Gold, Silver or Bronze award.
